Service Provider Application

Roadside Service Providers such as Towing Companies, Roadside Repair and etc, please fill out the Independent Service Provider Application Form below. This form provides the necessary information required by RoadsideMASTERS.com to qualify you for the Service Provider program, only vendors who provide the necessary information in the form, and qualify will be contacted by RoadsideMASTERS.com.

Being accepted as a service provider, you understand that while RoadsideMASTERS.com is responsible for payment for services rendered, you are an independent contractor, and have no affiliation with RoadsideMASTERS.com.



Insurance Requirements:
In order to keep you in our dispatch system, RoadsideMASTERS.com must have on file at all times, a copy of your current certificate of insurance. Please ask your insurance agent to list RoadsideMASTERS.com as a certificate holder, and submit your insurance certificate using the Vendor Portal.
